Antenatal shared care

Antenatal shared care is an increasingly popular model of antenatal care for women with low-risk pregnancies. Antenatal shared care involves GPs with additional training in women’s health and pregnancy working in collaboration with a hospital-based team including midwives and obstetricians. Shared care provides women with excellent continuity of care with your doctor and greater access to your doctor. As well, once the baby arrives, the same general practitioner that saw you through your pregnancy can provide continuity of care through assisting with baby health checks and attending to the health needs of the baby.

A government funded comprehensive care plan for chronic disease management is tailored to your specific needs to help you achieve your goals. The care plan is coordinated by your GP to manage all health care professionals involved in your care. If you have a chronic condition such as diabetes, hypertension, osteoporosis, arthritis, stroke, cancer, chronic pain please enquire with your GP if a care plan may be suitable for you. In addition five Medicare-subsidised allied health (e.g. podiatry, dietitian, physiotherapy, chiropractor, diabetes education) visits a year to support your goal attainment.

Health Assessments

A health assessment for those aged over 75 years is an in-depth annual assessment of physical, psychological and social-wellbeing and offers preventative health care and education. Our practice nurse conducts this assessment and this is followed by a consultation with your GP.

A health assessment is also offered for those aged 45-49 years.  This assessment aims to assist in early detection and prevention of chronic diseases. It enables prevention strategies and involves taking your history, noting your current health issues and lifestyle risk factors(such as smoking, nutrition, alcohol consumption and physical activity) as well as family history.
